Idiopathic intracranial hypertension in the emergency department?

John de Witt asked:

Is anybody else diagnosing stuff like this, or am I just a sh*t magnet?
Tonight’s was a chubby 15 year old girl without visual disturbances (and of course with sharp discs).

A Long Time Ago, I Had a Patient With Sudden-Onset Psychotic Sxs., I Traced it to the Cipro, it Resolved.

Can you describe her presenting symptoms and how you made the diagnosis? Also, are you seeing more of this in the ER in recent times?

Not personally, since the last time I did a funduscopic exam was about 2 years ago. But since IIH/pseudotumor cerebri is strongly associated with obesity, and since obesity is on the rise (starting at even younger ages now), it doesn’t surprise me.

I’ve heard about that more recently, so I don’t think it’s you. (But you still may be a shit magnet - let’s not rule that out!)

I know it’s something we have to watch out for more pre-op, especially in young overweight women. There are case reports of them waking up blind after surgery.

Have you been watching those TV doc’s again?
I liked the episode of House, that had the guy who had had a childhood disease that didn’t present for 30 years.

So you diagnosed a chubby girl with sharp discs and no visual disturbance with IIH? How did you come to that conclusion? I find this very interesting.
